PURPOSE Pilot project to establish community-wide case conferencing for the coordination of interorganizational care across medical, behavioral-mental health, and social services organizations- for clients and with clients in Southwestern Colorado. WHEN The pilot project ran from October 2014 through March 2015.
All Meetings Followed Four Simple Steps
1. Fill out SUGGESTING A CLIENT. We will then send email invitations to all who should attend the case conference to solve a care management problem for the client. If you suggest a client, please invite the client and facilitate his/her attendance. Clients may call into the meeting toll-free: 1-800-832-0736, room # 594240.
2. Please bring this Confidentiality Agreement with you to your first meeting.
3. After a release of information, HIPAA privacy agreement, and client consent are verified, the case conference will begin.
4. After the meeting, the group will together craft a brief summary and mail the NOTE TO PROVIDER to the client’s primary care provider.
