Accountable Health Communities (AHC)      Currently accepting additional partners.     

Goal: Addressing health-related social needs through referral and

community navigation to reduce healthcare costs and improve quality and delivery.

Focus: Community convening and clinical workflow

Community Resource Network

Goal: Care coordination for health improvement through client empowerment, navigation & access to services, and point-of-care support. Better community relations through a network of equals for open and efficient communication across systems.

Focus: Health information exchange (HIE) and social information exchange (SIE) aided through EasyCare Colorado (see below).

Focus: consortium of La Plata County service and faith-based organizations that provide services to those in need in our community; for example, convening/education/training to address suicide prevention or social isolation.

Mission: To empower individuals and families to achieve and maintain physical and mental wellness through peer interaction and education in a supportive and safe environment.
